The difference between

 <?foo ...?>

and

 <?xmlbar foo ... ?>

where 'foo' by intent is a name in a notation declaration, where this
syntactic understanding is captured in a formal specification of the
meaning of a reserved word such as 'xmlbar'.  See, e.g.,
