a) keep the namespace the same (don't want to lose the honour of having
   a "1998" one:-)

b) Add a version attribute to <math if we ever have a version of MathML
   where we think its needed. (Actually we should probably have
   had a version attribute anyway, although currently it's not specified
   that way.) (Do as I say, not as I do)
