You may also want to try a trial version of the latest Antenna House release.

This might or might not relate to your problem, but the fixes for AHF 6.1
MR2 includes:

   FO block in a multi-column area that has the properties
   keep-with-previous="always" and span="all" was broken
   across to the next page.
