Am I right in saying that even for a dynamic solution, xsl:namespace
is always only necessary when we are talking about namespaces of
content of elements or attribute values.

We can always replace a literal element and attribute with xsl:element
and xsl:attribute and both the name and the namespace attribute of
those elements are attribute value templates.
