It appears from your use of attributes that it is operating correctly.

      <xsl:template match="//SelfStudy" mode="toc">
      <fo:block xsl:use-attribute-sets="TOC1">
                        <xsl:text>Self Study&#160;</xsl:text>
                                    <xsl:number format="A"/>

That only counts the current node amongst like-named siblings.

However, I would like it to output as follows:
   SelfStudy A - Title
   SelfStudy B - Title
   SelfStudy C - Title

The following will count all SelfStudy elements to the beginning of the document:

<xsl:number format="A" level="any"/>
