What is the result when you drop the 'exclude-result-prefixes' attribute on xsl:stylesheet?
I see no effect on the result when I drop this attribute.

Sorry that I can't really help any further. Have you tried using non-namespace specific algorithms like xsl:copy-of select="@*"? And what happens when you change the default namespace in your xsl into a named namespace?
