This is the first time you give us an indication on which input should trigger another output.
Still not too clear though, as David seems to have missed it in your post.

So you could use the two-templates code David suggested, with a check on contentFieldStyle:

 <xsl:template match="contentField">
  <span class="{concat(contentFieldElement, '_', contentFieldStyle)}">
   <xsl:value-of select="contentFieldValue/contentItem/contentItemValue[@type='text']"/>
  </span>
 </xsl:template>

 <xsl:template match="contentField[contentFieldStyle/. = 'A']">
  <a class="{concat(contentFieldElement, '_', contentFieldStyle)}"
href="{contentFieldValue/contentItem/contentItemValue[@type='url']}">
   <xsl:value-of select="contentFieldValue/contentItem/contentItemValue[@type='text']"/>
  </a>
 </xsl:template>
