loop through the attributes using an xpath expression to get the value of
the parent instance.

i.e.

<xsl:for-each select="Your attribute path">
.......Your code to write your item identifier element....
your title would be <xsl:value-of select="parent::INSTANCE/@name"/>

I think that would work....
