You can't reference an XSLT variable unless you use it in an expression, so
try:
<xsl:when test="$vtype = 'num'"><xsl:value-of select="$vlevel +

<xsl:otherwise><xsl:value-of select="$vlevel + 3"/></xsl:otherwise>
