Hello!

That's because you are creating select element for each answer element - children of question. 
Create only one and multiply options instead of select:

	<xsl:for-each select="question/answer">
		<option>
			<xsl:value-of select="@match"/>
		</option>
	</xsl:for-each>
</select>
