The cited page-number string is obtained by converting the
  cited page-number in accordance with the number to string
  conversion properties of the ancestor fo:page-sequence of
  the referenced formatting object.

It looks like the software implementing <page-number> is respecting the format token of the <page-sequence> as it is supposed to.

This would indicate either:

(1) - the format token of the page-sequence containing the block you are citing for your last page is not "01"

(2) - your software is not respecting the format token of the page-sequence of the citation as it is supposed to

I can't think of any other reasons.
