You don't--use

<xsl:sort select="*[name() = ../../SortDetails/SortField]"
order="ascending"></xsl:sort>

or
<xsl:variable name="Variable" select="PatientList/SortDetails/SortField"
/>
<xsl:for-each select="PatientList/Patient">
<xsl:sort select="*[name() = $Variable]" order="ascending"></xsl:sort>

<td width="17%" height="30"><xsl:value-of select="LastName"/></td>
