Thanks for pointing that out - I changed to a URI but no joy.
In the end I found that I can tell the XMLDOM object not to validate the XML
source by setting its validateOnParse property to false.
Which seems to work.

that is incorrect syntax (for XML, irrespective of XSLT) as the SYSTEM
is supposed to be a URI not an MSDOS file path, so
<!DOCTYPE Hierarchy SYSTEM
"file:///C:/Program%20Files/IMS%20HEALTH/Dataview%206/DVH.dtd">
