not sure where the Master List of Things To Fix in 2.0 is kept
(the spec appendix is just a snapshot), but i'd like to add two
small things, motivated by some wasted debugging time this afternoon:

- a xsl:stylesheet attribute to disable built-in rules

- a xsl:stylesheet attribute to set the default input document namespace
for use in expressions, distinct from the default output namespace
