Can anyone suggest how to force a line break in XSL FO? the equivalent 
of <br/> in HTML? I cannot, after all, directly create a `line area'.

No, don't say `we decry visual formatting', this is implementing
markup which describes an existing historical document.

And what about marginal notes? Are they in the spec? I can't see at
first sight.
