Subject: format and initial-page-number attributes of fo:page-sequence

The format and initial-page-number attributes of fo:page-sequence seem
to have exactly the same purpose (to define the first number in the
sequence). I suspect these should be merged. For compatibility with
XSLT, I'd suggest deleting initial-page-number.
