 There is a very big example of something akin to incremental 
 transformation in wide use: the SmartArt in recent versions of MS Word 
 is stored as a 1-4 level list then transformed (as XML by XSLT) into the 
 visual form. (This probably isn't what people are meaning by incremental 
 XSLT, but it is a branch of the larger notional XML document being 
 transformed.)  I think MS have a patent on some part of the techniques 
 (Brian Jones, IIRC)...
