The best I have seen is Xopus (http://xopus.com). It is what we use for 
the editor in our CMS. It is the closest to Word that is out and lets 
you create whatever markup you want (as long as a schema is defined for 
it) - IE only. XStandard (http://xstandard.com/) is the best XHTML 
editor I have seen and allows you to create a psuedo schema. XStandard 
uses a plugin works on MS in IE and Mozilla and provides a really nice 
UI, but only creates XHTML.
