I'd say that the many excellent questions you raise are enough to revive the
dormant xml-hypertext list
(http://lists.usefulinc.com/mailman/listinfo/xml-hypertext). 

When it was rolling, we didn't take the "hyper" part of its name too
seriously, discussing many other linking issues, like the ones you raise, as
well.
