* Topic 2004-0004: xmlns Namespace

- no need to put xmlns:prefix attributes into a Namespace, as long as
   attribute declarations are reported (Elliotte Rusty Harold,
   2004-02-18)

- add paragraph from CVS Attributes.java about xmlns-uris feature into
   main release (Norm Walsh, 2004-02-19)

- clearly document interaction between xmlns-uris feature and
   namespace-prefixes feature; revised text on offer (Norm Walsh,
   2004-02-19)
