I've chosen entities mostly because there's a tremendous amount of
possibility here - lowest entry cost (now that I've written a parser,
anyway, though I think libxml might provide similar access), greatest
possibility for making my own work life simple.

I suspect that a lot of the challenges around entities have to do with
the question of what constitutes an advance.  It seems like a case where
coming up with a sample processor is a better way to gauge people's
opinions than discussion, so I'll see what I can come up with.
