I've had a fair amount of success with such tools, but to me, success is mostly defined in terms of reduction of effort, not perfection :-) FWIW. I've seen a lot of people use Frame+SGML, and Interleaf, some folk with WP+SGML and I've seen people deploy Word-based systems with style consistency checking. In every single case, the most important things was being able to gracefully handle the exceptions, because there are *always* exceptions!
