If attributes are defaulted to the element's namespace in SAX, 
and you build a DOM tree of the parser output, access to 
unprefixed attributes will be unclear in DOM:

call getAttributeNS (null, ....) or getAttributeNS (elementNS, ....) ?

Also, a DOM writer that follows the namespace spec, would add 
prefixes to all unprefixed attributes (!!)

Thus, either the namespace specification should be simplified or
unprefixed attributes should not default to the element's namespace.
