Either will do: given that end-tags cannot contain attributes, and
that an element has exactly one start-tag (not true of SGML, BTW),
an element contains two conflicting attributes iff its start-tag does.

No.  An attribute not explicitly labeled with a prefix belongs to the
same namespace as its element.
